I've used it both in normal position and also with it towards the body. Cat much prefers it worn backwards towards body and it still does the job that I want. I put this on and was surprised that it seemed to be quite big, thirty seconds later the collar was on the floor and the cat was licking her wound. It might be xs and for cats but you would need to have a pretty large cat to fit this collar though. I bought this one because it came the next day, rather than because I wanted my cat to look like a frog, it did make me laugh for the brief moment she wore it though.
