The medium felt too loose on me, especially as the bottles started to fill up, but the small was too small. It tended to skip down while I was pumping, meaning I had to use my hands to support it.... Defeating the purpose of a hands free pumping bra. It's just not adjustable enough to be a good option for everyone.The simply wishes bra has straps and a large Velcro section in the back, which allows for a custom, adjustable fit. I love that it is a swimsuit type material and stretches easy to fit different types of flanges. This is the first pumping bra I've purchased and used so I don't have much to compare it to, but for me it's effective. I am able to maneuver and work while working and not have to be worried about holding the pumps on or God forbid lose any milk due to accidentally dropping a pump. My only negative is that the material the eye hooks are attached to have hard corners and they take a while to be "worn in".
