You start off with 30 minutes and build up to about 3 hours. **Q: Mine didn't come with a guide and it's really uncomfortable. The back digs into the back of my neck. What am I doing wrong?** A: Mine came with a guide but that didn't help. You are supposed to use it for 30 mins a day for 7 days the increase 15 mins every couple of days. It help with my shoulders but hurt my neck and back. So I return mine. Hope this helps **Q: How close is the fit to the back of the neck? I sit at a desk all day and I have done so all of my working life. I hunch over the desk and within a few hours I'm dying of pain. It's so hard for me to sit up straight at a computer desk. This really seems to help!!! It is a little uncomfortable and makes my back a little sore (when I take it off) where it normally gives me issues. It's a good sore though. I can tell it's working and doesn't hurt that bad really. I've only been using this maybe a week, if that. It makes me sit up in the correct position and doesn't allow me to slouch. I can tell when I take it off how much it has helped. My neck/back/ shoulders feel so much better and relieved. (Even with the soreness which isn't bad and doesn't last real long or hurt anything like the normal daily pain) This also trains you to sit up straight when you're not using it because it makes you feel so much better that you want to sit up straight. I didn't realize how much I need this in my life!! Best purchase ever! It also helps my posture away from the desk. It is a little difficult to get it as tight as needed on your own. I finally figured out a way to do it though. I'm small. I'm only 4"11 about 100lbs. This fits me great and has room to tighten more if needed. I think it would work on someone average size also. It's has some pretty long straps and it's adjustable in a few places. I can say after a week of having put this on it is helping me to stand straighter after I take it off, so I can say that this really helping me with slouch issues.
