The strap isn't long enough to go around your waist like a belt (unless you're really, really trim), but it does an excellent job of wrapping around arms, forearms, thighs, calves, ankles -- and you can get creative draping the cold/hot pack over the back of your neck or over one shoulder and connecting the straps under your opposing arm. The only concern and the reason that they're not getting five stars is that they are a bit overfilled. At least the ones I received. If you're using them as a heating pad just be aware that they pop pretty easily. I had one out of the two in the pack pop on me pretty soon after purchase, it was at night, it was gooey, it was not fun. So if you're just placing the gel pack somewhere it's fine. But if you put pressure on it, like you have it on your back and you're sitting back in a chair it it has a high probability of popping. Unfortunately the seams just don't seem to be too strong.
