You can also wear it over heavier clothing as well. I always wear something under it to shield it from my body as well as for comfort. It's a great brace I feel, it does what is says it will. **Q: Is this more for lower back pain or pain in the upper part of the spine?** A: Well, to be honest I haven't ever worn it because of the fit and also because I have lower back pain and this was more for upper back. The straps were uncomfortable and very noticeable under clothes! The straps dug into my arm pit and sides. It wasn't work it.
