The back strap is adjustable so you can figure out how tight you want it. One of my ears, the right one, is hard of hearing so i position the headphones so the speaker is directly over the right ear but offset for the left ear, and that works very well to balance the sound output. it works without a battery, so i do need to crank up the volume on my iphone to hear well, but once i figured that out, the sound was good. One last thought - i have a cpap pillow and that helps when i am on my side so that the speaker does not smush into my head. with a regular pillow it might be a little uncomfortable. (a cpap pillow has a hole cut in the side of the pillow to allow for bulky items e.g. i use the contour cpap head pillow by contour products (no affiliation).) good luck!there may be cheaper products out there, but i do like this one. ### ⭐⭐⭐⭐ 4.0 out of 5 stars Good sound and solid construction *by C***E on Reviewed in the United States on June 20, 2016* Good sound and solid construction ..The mask is bulky so not too portable for plane trips which is why I bought it.
