How long is the hose and can you shut the water off at the shower head to save water while showering?

Yes you can shut off from the shower and is about 6’ long.

what type of hanger does this use for the wall?

It sits right where you see it in the product picture, between the two handles. Wall fixture not needed. But since we had one in the shower from an older fixture, sometimes the pull-out hose gets draped over that. You could certainly add one of those self-stick hooks to the wall and hang the hose, but it doesn't need it.

Is fittings sad or metric

How is the shower setting. is it a good spray? Does it stay on the spray setting or do you have to hold it?

I just replaced mine. Had this product for a long time. The spray nozzle is great. But it all depends on your water pressure of course. At the end of the faucet you can either pull out the head for shower spray, or push in for 1 stream. It clicks in and out well and never loss it’s position. The single stream is good for regular sink use. I’m happy with mine.
