No drilling needed. If it helps, I buy TP from Who Gives A Cr*p (it's a brand!) and the chunky rolls fit absolutely fine. As for stickiness, the photos are of one I put up without any screws (see photo) and then tried a 2.5Kg weight, then a 5Kg and both aren't making it appear that it's going to give way, so a small box, spare roll or fragranced diffuser will be absolutely fine. Maybe just make sure the surface is flat enough and clean with a degreaser first to be sure (I didn't). The roll holder arm feels beyond sturdy enough.
