It's quite tricky to get threads aligned properly. It's counter-intuitive but it's best to hold the glass jug upright with one hand and screw the black cutter assembly in with the other hand i.e approach the jug from the bottom. Once you're confident the threads are engaged properly you still need to screw the cutter assembly quite tightly and then when you operate the blender again you have to turn the jug quite tightly to make sure the safety interlock works.
