Review: Simple and effective way to stay organized - This personal health record keeper works great for keeping track of important information all in one place. It’s a simple but very useful tool for organizing medical history, appointments, medications, and other health details. The layout is easy to follow, making it quick to write things down and find them later when needed. It’s especially helpful for staying on top of ongoing health information without having to rely on multiple papers or notes. Overall, it’s a practical and effective way to stay organized and keep everything easily accessible. Suggestions for improvement: Add tabs or sections for quicker navigation Include a few extra pages for notes or updates Offer a digital companion or printable backup option
Review: Good little record book - I am the "secretary" for all things medical for myself, my hubby, and my mom, so I have to keep decent records to keep things straight. For several years I've just used a spiral notebook and tried to organize it to accommodate the info I need to keep track of. This little book just makes it much easier, with a place for all the different kinds of information I need, from doctor's names, phone, address, lists of immunizations, a place to record procedures done, plenty of room to keep track of medications, and then a two-page spread for collecting info from each office visit. It's also smaller than the spiral I was using and easier to tuck into a bag or purse. It even has an elastic band bookmark so you can mark your place for the next office visit. The only thing I'd change is to put a loop for holding a pen (because I never have one when I need it!) and maybe a pocket in the back for holding the appointment cards or important printouts the Dr. gives you. It's not perfect, but it's a pretty useful little book that does what I need it to.

### ⭐⭐⭐⭐ Close but no cigar

This book is terrific, and I am very happy I bought it (why did I never think to look for something like this before). That said, there is definitely some room for improvement. Add bookmarks I’m 25 days into a new year and I’ve already had 6 appointments due to a fun dental situation. Having to page through the book to find my notes on the diagnosis and the next upcoming appointment is really annoying. I taped 4 pieces of ribbon in the back of the book that I’m using to mark certain spots. This is something that should come out of the spine so it isn’t a big loop over the top of all the pages (or pulled out every time you open the book, if you cut the ribbon too short). Reformat the Vision Health page Everything about this page is a little weird, honestly, and makes me think someone who does not wear glasses designed it. I have included a photo of my attempt to list the results of my last eye exam. Why not reduce the number of tests you can list, and reformat the page to actually look like what we get from the doctor? The notes section takes of half the page, which just seems like it was filler added because the number of lines for exams is already enough for a decade’s worth. How about 3 lines for each visit: left eye, right eye, date of exam and notes? Add a set of teeth to the Dental Health info First, one page for dental health notes formatted I’m taking a short answer essay test is not going to do it for me, after major surgery on my jaw that requires lifelong follow-up, TMJ and gum recession, and many root canals and fillings from over the years. Why not add that fun diagram of your teeth the dentist has, so I can make notes on where things are and what problems they’re having and what still needs to be done… on single page versions of the appointment records. I’m using those pages and half the info is totally irrelevant to the dental health visits. Insurance and Pharmacy info needs notes too Due to supply shortages and supply chain problems, CVS won’t fill this prescription from that provider and Walgreens won’t fill it without a local address on the script but CVS Caremark is my insurance and etc etc. I seriously need a daily log to keep track of this nonsense. Why is healthcare so hard? (I live in America. That’s why.)
