I look for the shortest distance which is the back of the lower jaw under my ear and measure straight down the the clavicle making sure you head is held straight. My neck feels nicely supported when wearing it and provides me with great pain relief. I can comfortably wear it to bed without any discomfort. I purchased a Small even though I was between two sizes and the brace fits perfectly. Love the removable cover which is very soft and silky. I'm only giving 4 stars for comfort because the Velcro is new and very tight. Hoping with use it will be easier to release Velcro.
