I have one with a metal insert that is torture!** A: No you can't use this as a splint for achilles tendonitis but for achilles tendonitis I do know that a pressure boot is good where most of the pressure comes from the back of the ankle and around the ankle and it is adjustable. I wear mine on my right foot and have no problem. I also have a very ridgid metal one that clamps around my calf, which is very uncomfortable, hard to put on/take off, and interferes with driving. This device has some give which helps activating the throttle, but keeps me from tripping and falling. I'm on my second one and am very pleased with it.
