You control how much pressure you apply. You can gently twist or slightly move the handles in/out for a "kneading" massage sensation while applying pressure. The handles have finger grips - if you use it with the finger grips facing up it works great for the neck area; if you use it with the finger grips facing down it works better for the shoulder area on either side of and below the neck. A quick 5-10 minute massage in these target areas works great for me to loosen things up and reduce my pain so I can continue on with my day. For me, this was worth every penny!

I have extremely bad neck and shoulder pain due to spine issues and needed something to help with muscle tension/pain in between my therapy sessions and this does the trick. Fast shipper. Product works very well. My sore neck muscles are greatly relieved by the use of this massager. It mainly works the mid to upper part of your neck--the bottom of your neck is difficult to adjust the size for. I tried using it on my shoulder but couldn't get it to fit. The only reason it is not 5 stars is the ball material is a little "sticky" and pulls my hair if I don't have it completely out of the way (which is difficult in the neck area). I use this almost every night and sometimes in the morning. Big difference. I do recommend this product.
