The wrap is just foam fabric and Velcro. The ice pack is gel and very well contained and has been long-ladting for me. What is an extension strap?** A: I’m pretty sure as it measures 55 inches from end to end and the straps are stretchy. The only problem I had with it was the bands stretched with each use and the Velcro stopped working well after a couple of weeks. Also the ice pack inside moves a bit inside the mesh pocket.
