It has good massage and heat but it is a little heavy.I live alone and am 76 yrs with arthritis so I may send it back **Q: Are there heat coils in the neck of this item?** A: Although there is heat there not sure how they did it but you can not feel the coils anywhere in the heat pad. I love it, it is great!! **Q: What is the moist heat option?** A: No **Q: Does the heat in the neck portion heat all around and to base of neck and head?** A: If I lay against a pillow wearing it, like propped up on the bed, I can feel the heat effectively around my neck area and shoulders. This pad has heat settings from 1 to 6, and six feels really good if you're aching. It doesn't burn me. It also has a time setting of 30, 60, 90 and 120 minutes. I have only worn this pad in this position, as it's near my bed. I think the heat would be adequate if you happen to sit in a chair and wear it. It does bring comfort as I bought it for my arthritic shoulders. I do feel it well on the neck.
