My last order on May 12, 2018 consisted of 12 chips. thus far at least 4 have failed. I build small audio DI units for guitar players. My delivery schedule has been severely delayed due to this situation. My oscilloscope shows the same malfunction on all the chips @ the same pin locations, bar one. Now I must purchase chips locally at greater expense. I can not trust the remaining five chips.
