I use it on the floor, as I sit and sleep on the floor these days (lifestyle thing and also don't have furniture right now). For sleeping, I use a yoga mat with this thing on it's long side with the short side against the wall so that I am sleeping lying down but on an incline for the top part of my body. I find this works for my breathing issues and is comfortable. I use a small pillow on top for side sleeping, otherwise there's nothing supporting my neck and my shoulders hunch up. (I also have additional padding to sleep on as well in this setup.) I especially love this for floor sitting and definitely recommend it for that. I put the short side on the floor and the long side against the wall. In front of that, I use a folded rug/blanket and large, flat pillow to sit on. When I want to sit up a bit straighter, I just add a small flat pillow behind my back. All set! Of course you can do all this on a bed just as well. I find the firmness and softness to be perfect.
