It is most likely a comfort issue rather than a functional issue. The knob end helps in finding the precise degree of pressure needed for therapy. The smooth end is usually for gripping, but I don't see why you couldn't use it for therapy if it is a better choice comfort wise. **Q: Which one does Amazon deliver? the Therawand or the thinner one?** A: The Therawand Premium is the only one currently being offered and that you are inquiring about. I've been stretching a lot more than I had been, and I started using a Therawand. When I opened the package, the diameter was less than I expected it to be, which is actually good, because it will be easier to insert and use if I am experiencing spasms. I suspect the small diameter would make it more comfortable for a man to use to treat PFD than many dilators would be. Right away, it has been an excellent tool for reaching tight spots, and, for me, the curves make it easier to target trigger points than straight dilators. The only thing I am not thrilled about is that it is acrylic, not glass. It has a really smooth surface and is medical grade, so it should be easy enough to keep clean. I use it for my pelvic floor dysfunction 3 times a week to loosen an overly tight coccygeal muscle. It's very slender, so if you have total tightness down there, there's almost no pain with insertion. The handle design makes for a decent grip too.
