Yes it’s a shirt and pants. You wear them uber your clothes to keep you warm. My husband says they are awesome and keeps him warm.

What size should I order if my boyfriend is a large shirt but usually a small/med in underwear

Just my opinion, I would get a Medium-Large. Pajamas are usually big for their stated size; however, you want more of a tight fit with thermal underwear as to keep the heat in, and usually warn under regular clothing. So I would say medium. Everything fits differently, but I would say the medium would be a better choice under a large shirt.

this is fleece lined?

It is not "fleece" material - however, the warmth is similar to that but is instead a form fitting base layer. I wear these under pants during New England winter temperatures and it is just as warm as smartwool. I also wear them under my motorcycle gear during this season too. These are a great product and worth way more than $20!

I wear 36 pants L but the size chart wierd what size do i get ? the chart size 3xl that big ?im L 36 help ?

Im 36 waist myself but my upper body is xxl because im 6'1" so is the size i gotten and it fits snugg all the way around because its stretchy material and here in new england its really cold and im in the snow clearing team , i luv this material it keeps me warm and dry ,hope this helps
