How does it 1st cut the pills?

It doesn't cut them it grinds them into a fine powder. I use it to mix my dog's meds with her food.

Can it grind one aspirin, or similar size single small pill?

Yes. 1 pill at at a time. If it is a large pill I had to cut it in half, then grind it.
