does the storage case itself have individual slots that the coin holders sit in?

No there’s four slots they sit in, the only dividers is between the 4 slots, also I purchased this for challenge coins which do not fit, I believe Kennedy 1/2 dollars and possibly silver dollars did fit.

Will these fit a 3mm thick coin?

YES! This is a fantastic case to store your coins or rounds in. The coin capsules will hold 3mm thick coins depending on the coins circumference. I haven't come across any coins that were too big (circumference) to fit. Very good case with capsules to store your coins or rounds in.

This is the second time I ordered this. The first item was good the second one was missing 4 capsules and the case was broke. Please let me know how

Are these flat on top and bottom?

First of all, they are flat on top and bottom. There is a top and a bottom because you can tell the bottom piece has a recessed ring so that the top fits over it. They are high quality, fit tightly, and I would recommend them.
