Is there any way to lock/attach the rack to my hitch to prevent someone from simply unscrewing the bolt and stealing the rack?

Yes, get The MaxxHaul 50563 Threaded Hitch Locking Pin. Works perfectly with Yakima double down bike rack

Will it actually hold 4 four bikes for sure?

We have it on our minivan and it was tight with 3. I would definitely be concerned with 4. As it was, my chain got pulled and one of the bikes got scratched from putting on 3. We have 2 mountain bikes and a cruiser. You will need to purchase additional items if you have a cruiser and a woman's bike.

Would this fit on a 2012 Subaru Outback?

Yes, this would fit, but you would need to have the appropriate trailer hitch installed. I have this rack on my 2004 Forrester, and I had the hitch installed at U-Haul for about 200 including parts and labor.

As of 2018, does this still only hold 3 bikes?

Tough question ... there are straps for 4 bikes on the Yakima that I bought, but I, personally, would have a hard time fitting 4 bikes on it -- I've only done 2, and think 3 will work, but be tight.
