Would these work for cauliflower that formed just outside the ear lobe?

Not sure. Mine formed on the inner ear.

How does it feel wearing these under headgear while rolling?

they'll stay on your ears if you're not messing with them, but even taking off my t-shirt makes them come off. might as well just put them back on when you're done with the headgear, and finished for the day.
