No. It's exactly like a regular pad, only it doesn't have a sticky stripe and it is cool for about 20 minutes.

Do these have self adhesive strips on them?

No, it does not come with an adhesive strip. I received a pacakage that has a green plastic cover and it doesn't come with a self adhesive strip. The image shows the product in purple plastic package, identical to the ones I had received from the hospital and those had a self adhesive strip. Having used one with adhesive strip (from the hospital) and one purchased from Amazon without the adhesive strip, I would say that the adhesive strip would not help much because it still would move around a little bit. Hope this helps.

How long does the product stay cold after activation ?

I usually apply it at bedtime and I'm able to fall asleep using it. I'm guessing it stays cold about 2 hours at the most. I usually don't need to use another one during the night. They work really well for me.
