The picture shows the hardware mounted with the open end on facing right. Can it be mounted with the open end facing left?

Hello, While this toilet paper holder can be mounted with the open end facing left and will function perfectly, please note that the tiny set screw would then be visible at the top edge of the base plate, instead of hidden under the bottom edge. also, the faintly-printed brand logo will be upside-down (although better hidden by the post).

Will this fixture accommodate a large roll of TP like the ones from Costco?

no, it fits the more standard size rolls.
