Do i have to take it of during shower ?

I don't always take the tape off when I shower. If the edges are curling or I've had it on for several days, then I do.

Would this help smooth out wrinkles?

I really doubt it! It's made for scars, superficial skin issues, not deep tissue matters like wrinkles.
