The Hoverpen comes with 2 cartridges, one is fine (in the pen) and the other is medium point (in the package).

Is there a big difference between a Silver and the gray color? The gray just looks more like a matte silver in the picture.

Hello Marcella, I took a vid with my phone for your reference. As you can see in the video, the silver (pearl gray) pen is almost the same color as the silver pedestal, while the dark gray one is much darker.
