They are all the same size. There are 60 pairs foam ear plugs and one pair silicone ear plugs in the package.

Almost certainly not. The writing on the image giving instructions for how to insert the earplug was clearly written by a non-native speaker.

Do the bottles have plastic over the lid or a seal under it for safety?

I cannot remember if plastic was used over the lid. It is a screw on lid, with a cardboard sealer in the lid. Maybe the manufacturer could advise you. I do know these earplugs are awesome for cancelling noise. I had bought a quality pair of noise screening earphones and they did not do the job. Tough to screen the noise of a trampoline over your head.
