DO NOT USE FOR SHINGLES RELIEF!!!!
I looked on the inner-webs for a pain cream to use on my Shingles outbreak I am currently going thru on my neck, chest and back of head. The GOOGLES recommended Capsaicin as one of the highly suggested pain relief remedies. FU GOOGLE! Talk about FIRE PAIN! NOPE. Never again. Put this stuff on, slathered it up real good... Pain went from 50% to 1,000% in about 30 seconds. I tried to lay down and rest thinking it would be like Icy Hot (Hot then Cold then Hot then Cold then Hot.... You get the picture.) I was WRONG. Had to wash this stuff off with soapy water and a wash cloth. I thought I was peeling my skin off as I wiped it hurt so much. Lesson learned here - NO CAPSAICIN if you have Shingles rash and blisters. You are welcome.

Good for warming feet, no noticeable pain relief
The amount of product and capsaicin is good for the money, it takes about 10 minutes to feel anything noticeable. All you'll feel is a bit of heat, nothing extreme, I even tried applying it on softer tissues like on my lips, nose, and around my eyes and it only provides heat for ten or so minutes before dissipating. You're best off using a hot towel on the area for a few minutes to open the pores before applying the cream, and if you want the effect to last you need to cover the area with bandages or cling wrap.With that being said it doesn't really cause a drop in discomfort or pain, but, it will keep your feet and fingers warm in the winter. I like to put it on in the morning and evening so that they stay toasty and don't get cold, as an added bonus wearing gloves and socks/boots helps prolong the warming effect. As a downside, the cream itself is VERY greasy and will leave a film even if you spend a few minutes massaging it into the applied area.
