What is there a minimum height the unit needs to below the shower?

For waste water from the shower and bath,
ensure that the slope of the siphon is at
least 1% (1 cm per metre), so that the waste
water flows through the natural gravity to
the unit position

Unit has started running continuously even when no water going through until presumably the thermal cut out causes it to stop... Why is this?

Because the build quality is terrible. 
Mine stopped working for the same reason after only about 6 months. Don't even try and get support from the company as they will wriggle out of their liability as they refuse to honour guarantees!
Go and spend a bit more money with a reputable company.

What can I can use to connect a trap to the inlet?

Connectors are supplied, but I cannot recommend this company as their warranty service is dire. Mine failed after 5 months and they refused to replace it under warranty due to restrictive terms in the fitting instructions.... Caveat Emptor!

suitable for washing machine?

Hi - I bought this for a washing machine and whilst the pump seemed to work OK it didn't come with all the things you would need for fitting - we had loads of trouble getting it connected and then there either wasn't a valve, or the valve didn't work, to stop the backflow which meant the pump kept having to switch on. It also leaked at the seals. In the end we bought the saniflow again that we had just taken out. It was a lot more expensive, but we'd had the last one nearly 15 years and had no trouble with it. The saniflow went in like a dream and I'm sure will be no trouble for the next 15 years - so in the end worth it. This pump OK if you employ someone who knows what they are doing and you get all the accessories - but mine ended up at the tip.
