The height doesn't determine when the mushroom is ready, it is the cap size. Once the mushroom caps start to curl or reach 1-2 inches in cap size, harvest all the mushrooms at once.

After a month all my mushrooms just shriveled and died, and now they have a bad smell. Is there any way to salvage them?

Did you wait too long to harvest them? When the cap flattens out and they begin to get hard, cut them all out, even the little ones. You only have a day or so in this window. Di you let them get too wet? They like it dry, and I am always amazed how really dry they like it.
