How can you tell what size is the right one for you?

There are three styles of our elaborate neck brace, namely comfort version, dual use and enhanced version. They are available in different sizes. Please be sure to measure the neck circumference and neck height and purchase corresponding products to achieve the best effect. 
Small：Neck circumference 10 1/2"-11 1/2"；Neck height 2 1/2″ & 3 1/2″
Medium：Neck circumference 11 1/2"-13 1/4"；Neck height 3" & 4″
Large：Neck circumference 13 1/4"-16 1/2"；Neck height 3 1/2" & 4″
X-Large：Neck circumference 16 1/2"-18 3/4"；Neck height 3 1/2" & 4″

Does this keep your mouth closed at night?

I wear this cervical collar (comfort version) every night to reduce my sleep apnea.   Even though I also wear a full face CPAP mask which covers my nose and mouth, if I forget to wear this collar overnight, my AHI (hypopneas per hour) are 15-18 (according to my CPAP machine's sensors), but if I wear this collar, AHI is only about 1-2.   The more I tighten the collar, the lower the AHI.   This collar keeps my jaw and mouth from gaping wide open, but nothing outside of a vise could completely prevent you from opening your jaw when you are asleep.   Believe me, I've tried every style of collar, chin strap (including homemade), and even taping my mouth shut with medical tape.   Try any of those things and you will see you can still open your jaw a bit especially if you tilt your head back a little.   This collar does reduce it enough to make a big difference in apnea.   I found a 4" collar too high (made me bend my head back a little and still wouldn't guarantee a closed mouth.  The 3" collar height was right for me.  Someday I might try the 3.5".

How many inches wide is this neck brace?  3, 3 1/2 or 4?

What is the extra tube-shaped piece of stretchy fabric that was included with my neck brace?

you slip it over the brace so that your neck and chin don't hit the actual brace.  That way you can wash the tube and keep the whole thing fresh.
