Does the bottle smell after first use?

No. It's the same as any other spray bottle.

Pull the cap off. It may twist off too. It is just like any other spray perfume bottle.

Can you carry the bottle in your coat pocket or does it stink too bad after first use?

I haven't noticed it being an issue. Iv'e sprayed it and it didn't cause any residual issues as far as carrying or storing. Regarding smell, I would liken it to a potent stock yard, if you've ever driven by one, you will know what I mean. It will get your attention and anything the spray lands on will carry the odor.

Are there any labels within the box that would obviously indicate its a gag?

No, it is pretty legitimate looking. It is only when you bring the box to your nose does a slight whiff of the foul sent even begin to indicate there is something not so real inside, but not enough to make anyone think right away it is a gag. We gave it at "what's that smell" theme party and it took a while for anyone to even realize what it really was.
