What is the max thickness that a door can be for these?

2" I used rough cut red cedar timber (a true 2x4) and it was a very snug fit and worked great.

Is having the hinge under tension required to have proper function? With no spring tension the door sags. Will it snug up with tension adjusted?

Yes Plus with no tension, you have no spring to pull the door back to its position in the door frame

ARE THERE TWO HINDGES OR ONE IN A BOX

One hinge in each box. I think they do this because quite often if you're using these with a full size door you may need to use three hinges due to the weight of the door.

How much room to I need to allow for the hinges when making my doors? These are double swinging doors so hinges will be on both sides

Half of an inch ; plus the gap between the doors..
