Is the interior surface smooth?

No, it is a wire spiral that supports the thin plastic wall. Inside and outside are the same.

If you just put the 5ft. hose on the floor, will it read 5ft. over do you have to pull hose to get 5ft?

5ft pulled out wish it was the other way around lol

Can I ground this by connecting to the steel wire that wraps around the hose?

Yes, I believe that will work fine.

Can this be utilized for range hood discharge? I want something smoother than flex duct to eliminate drag.

It's not smooth and it's not meant for hot air - I wouldn't use it for that purpose.
