Do you loosen the cap on top?

Yes, you must loosen the cap for the air to vent. If you don't it has no where to vent to. Loosen cap 1/2 to full turn. don't over loosen.

can this be mounted horizontally?

The air vent must be installed vertically.

Can this be used to purge air from a well water line (cold) after the pump and before it gets to the house?

Yes, but it should be installed at the highest point in the line.

Does Bignell know water is water hot or cold ?

No it is for steam it only works hot ..used to bleed the line of air so water flows evenly thru line
