Is the upper tier reversable (ie, to the left side)?

Yes. The Bottom Tray can be mounted in any 2 ways with the upper tray biased to Left or Right.

How wide is the top shelf?

The top shelf is 4.5" wide. It has a clear plastic liner too.

What is the width of upper tier? The garbage disposal leaves only an 8" full clearance between cabinet wall and disposal.

This has to be mounted so basket can slide out with the cupboard door open. On the outside of the rollout there is an upright chrome tube that extends vertically (it supports the upper basket). From that tube on the wall side of the basket, the distance to the wall is 1 and 3/4 inches. From the sink side of the basket to the cabinet wall, it is 7 and 1/8 inches. It took me two times to get the mount right. It is a well made roll out. I hope you enjoy the useful design as much as I have.

The pull out two tier sliding shelf is great, but does anyone know how to keep the black bumpers from falling off?

Bumpers on my shelf have not fallen off. Try using some heavy duty epoxy or chalk. May work.
