Can this be operated from above? I mean, if you were in the attic space and the unit/door is closed, how easy is it to extend the ladder to "go down"?

It cannot easily be setup to come down from the top. Door opens easily enough but the latter movement is almost impossible to extend. It’s not impossible but a special tool would have to be made.

I need a ladder to accommodate a 10'3" ceiling height can I purchase additional steps?

What you could do is build a box for the ladder to rest on. Think of it a 'pre-step'.
