Can an 18 x 18 pillow fit in this cover?

Yes, 18 x 18 can fit. They are really pretty pillow covers.

The covers are a thick burlap type fabric. The only macrame is what you see in the picture.. They are nice but very plain other than that small macrame flap.

Would a 20x20 insert fit to make them more full? Or is that too large?

At first I stuffed a 20x20 insert into them . Personally I didn't like it. It looked rigid and unnatural. I purchased an 18x18 insert on Amazon and love it. Looks like a down pillow more soft and natural shape

Love these pillow cases. They are very boho so they match my decor. I love the fringes and woven stings. I wouldn’t say they are comfortable, they are more for looks. I have sat on them, I didn’t have issues. But they are definitely made to look at. They are pretty thick, they have a good zipper. Value for the money is comparable to other pillow cases. They are just the cases you will need to purchase the pillow separately. The macrame is perfect. They are not stark white more of cream or off-white. I like them.
