Can you hook this up to your real doorbell so that when they push the button, your real doorbell rings inside to let you know they are at the door?

You can’t unfortunately. :( the reason why it’s because the doorbell itself works with (3 AAA batteries) 4.5 DC volts and your normal doorbell works with 24 AC volts. If you connect them together you’ll fry the microprocessor inside the Halloween one. It can be a way using a relay but that’s only if you know how to do it.

Is the eye closed until you ring the doorbell?

Create a spooky scene at your entrance this Halloween with a Animated Doorbell Eyeball. When activated, this Doorbell Eyeball decoration features a lighted green eyeball that moves back and forth. Made of plastic Sound and motion activated Measures 5" wide x 7" high

There are 4 different sayings: 1. In a creepy voice, You rang?, 2. Enter at your own risk with a long cackle after it3. Welcome with a ghoulish laugh4. Good evening in a count Dracula kind of voiceHope this helps.

How do you attach this?

There is a small opening in the back which would allow you to hang it on a small nail, screw or tack. *note mine is two old and the "lids" of the eye won't retract any longer. However it is very popular with kids.
