Can little people animals fit in this?

Sure, you can fit several in there.

Does it come with booklet

It comes with a book to order more horses, no instructions. We just looked at the pictures on the box to put it together.

Will spirit horses fit in these? They are 7" I believe

The Breyer Stablemates Deluxe Animal Hospital set holds Stablemates 1:32 scale horses. The Spirit horses that are 7" tall will be too large for this, however the Spirit Stablemates blind bags would fit with this Deluxe Animal Hospital.

Was going to buy this before i read all the bad reviews. Thanks to everyone who commented. Why is this made so cheap?

I bought one for my boys when they were 4 and 5 and still have it. They are now 10 and 11
