Decent but Could be Better
I use these at work and they get most of it right. I especially like the spot to sign for emergency services and matting. I also have several ideas:1 - We need date of birth, not age. Age changes. I don't want to keep filling out cards or asking my client to fill out cards When there's plenty of space on the back. If the owner doesn't know the DOB, I ask the pet's approximate age and write down a guess at a year of birth.2 - The "Allergies, Spayed/Neutered, Vaccinations Current" check boxes are too close to the wrong words. So many of my clients mark "Allergies" when they mean "Spayed/Neutered".3 - The "Allergies" check box is too vague. I need a space to write down what kind of allergies. Maybe "Medical Issues/Allergies:__________________" at the bottom of the section.4 - The "Vaccinations Current" check box means nothing to the health inspector. I need a space to write down the date the dog's rabies vaccine is due next. Maybe "Rabies Vaccination Exp. Date:__________".5 - Most of my clients under 30 don't have home phones. We just need "Primary Phone #" and "Secondary" or "Alternate Phone #". We only use 2 permanent numbers so we don't end up calling the dog's grandma or dog sitter when we're trying to call mom or dad.6 - The boxes around the words "Owner", "Pet", and "Groomer Notes" should surround the entire section. I think that would stop people from writing their pet's name where the owner's name should go. It's pretty common.7 - The "Clip" section is pretty useless for us. We put the clip the dog got on the back under "Services" for each appointment because most of our clients get different clips depending on the season, coat condition, etc..They hit the major points, though.
