How much cash will it hold? Will it hold $5M in stacks of 100 $100 bills ($10,000)?

I could only fit $4.9M. Unfortunately, it falls a bit short. I'd recommend printing $500 bills to fit all $5M

does it bolt to the floor or the wall?

The way these safes are listed on Amazon is somewhat convoluted, and the answers do not necessarily line up to the model you are considering. For model SFW123FUL, which I just received, it can be bolted to the floor but it DOES NOT come with the hardware included. You must purchase the hardware separately. However the screws, washers, and anchors needed to bolt this safe to a floor are very inexpensive and can be found at any big-box home improvement store. It was the rep from Sentry Safe who told me what specific parts I need, and he came on the line in less than a minute: 2 lag screws measuring 3/8" x 4", 2 washers, and 2 masonry anchors (I'm bolting down to a concrete slab). So, annoying that Amazon does not clearly distinguish what one model of safe comes with vs another, but due credit to Sentry's customer service.

What happens if the batteries go dead? Can you replace them from the outside?
