Tried 2, Neither worked with Eyefinity Setup
Tried two of these with my Sapphire Radeon HD6950 Dirt 3 Edition with a 3 monitor setup and it didn't work on any of my 3 monitors, or with my buddy's system either (same graphics card). Returning both. By not working I mean it had mixed results: The first screen I tried it displayed absolutely nothing (samsung syncmaster), the second screen was a hanns-g 1680x1050 native resolution basic widescreen monitor and it displayed full vertically but only 80% of the way horizontally, with a squished screen. Called Cirago--they had great customer service, picked up right away and had a pretty smart guy helping with technical support and told me to try another. They both did the same thing. Maybe my setup is wrong or something (I dont think so..) but I have newest ATI drivers, my setup is less than 3 months old, and the monitors work fine on basic DVI ports. I saw somewhere that you can use a generic (non-active) displayport to vga adapter and it should work just fine, so I'm trying that next.

Garbage
All the active adapters seem overpriced to me, so I thought I was getting a bargain with this adapter at less than half the price. Well, it looks like I got what I paid for quite literally: half my screen works at half my maximum resolution! This is NOT eyefinity ready. When I plugged in my DVI monitor,it said "input not supported." After trying it on every monitor, with several different cables, I was finally able to get it to display in my computer's bios. It was there I discovered that it still worked... a little bit. By turning the resolution down from 1920*1080 to 1280*720, I was able to get half my screen working on the right side of my monitor, with the other side being a large black bar divided by a small pink line. I can get a semi-functioning eyefinity setup if I use my crippled monitor output as a peripheral left-side monitor at 3072*768 while I wait for a real adapter to come in the mail.

Do not buy
I had the same results as others. This adapter does not work. Tried two different monitors. WIth an Apple Cinema display, it created a ghosting effect. On a Dell display, I could not adjust the resolution properly. On both displays, there was a pink line running down the left side. Do not buy.
