Is the inside waterproof?

If anything it is water resistant. I don't know that it is 100% water proof, meaning, I wouldn't fill it up with water like a canteen and expect to not make a mess. :) It's definitely capable of keeping any spilled or discharged liquids under control as the inside is lined with a nylon pouch.

Will it fit two blood pressure cuff, one adult large and one pediatric?

Not sure if they will fit properly

Is this real leather??

No, its not. But, its strong and my husband still has it in great condition.
