How long does it take from installing a new cartridge in a new pen for the ink to get to the tip. Is there any special way to place the pen to work?

It should take at least ten minutes or more for the tip to fill with ink and then use for drawing. There's no special way of placing the pen to make work, once the ink reaches the tip its ready to go.

Does this "Japanese version" use the regular refills?

It uses Pentel Arts Pocket Brush Refills. You can get them here on Amazon. If I use water color after outlining with this will it mess it up?!

I have just started using this brush for watercolor, and it does not mess it up! You can draw/letter with the ink before or after using watercolors with no problem.

Does it have extra fine or fine tip?

It only comes with one variating tip that changes ildepensing on how hard you press. If you hover it really lightly, it can be an extra fine tip
