Can you fit the end onto a drill in order to spin it through the clog?

No, the diameter of the rod is almost an inch. Now if you have a drill this large go for it and write me back. You have to use muscle power depending on your clog size and hardness.

Would it work on a 4” sewer pipe in USA?

Yes. Worked very well with my 4 in pipes.

will i get enough pressure to clear outside line

Not sure about question, but this only works in an easily accessible , straight , 4 inch pipe, like sewer pipe.
