Medicine History and Physical H&P and SOAP Notebook for a Perfect Presentation by Wellnote

Hands down the best book for SOAP notes I have found. As a student of medicine, I have to write down a lot of information and often have to sort it out into order to present it in a way my preceptor understands.The way this book is laid out is exactly how we are taught to do patient interviews and give patient reports. Has saved my life in time where I have done rounds and lost track of my flow or a pertinent piece of info. It’s so helpful I’m on my second book! Can’t recommend it enough!

Well-designed, and easy to use. Just keep in mind that the type is a little small, but once you are used to using this notebook, it's easy to record a systematic HPI, ROS, and physical exam. The design allows you to write over the lightly-inked labels for some of the entries, so there is more space than you might expect.

I’m a PA student on clinical rotations. I wasted a lot of pages (as in, lots of pages went unused) in this notebook because I ended up not needing the 2 extra pages for lab values, etc. I did like that the template was in lighter type so I could write over it, but the overall layout was also not for me. I think this is better suited for inpatient, and since I’m only on each rotation for 4 1/2 weeks, it didn’t quite fit my needs.

I ordered two of these books and love them! It took some time to get used to the layout of the pages but then it was great for clinicals! I was prepared for rounds since everything was right in front of me. It is somewhat difficult finding the patient you are looking for since there are no tabs, so I may just add small tabs to the pages so I don't have to spend as much time sorting through the pages. I am definitely going to continue using this book for clinicals.

Bought the notebook in order to get better at SOAP note writing as im a PA student currently. I haven't really used it much, mostly because there is only space for certain amount of patient encounters. What comes in handy are the normal values and proposed organization for a SOAP note. This is more on student preference but it just wasn't for me.

SO HELPFUL! I'm a Psychiatric Nurse Practitioner student currently doing my first round of clinicals. Diabetic checks, pre-op physicals, acute adult visits for physical health problems, etc. are not my wheelhouse. However, this notebook makes it simple to take down the information I need to consult with my preceptor and document my findings for class. I highly recommend!

I've purchased several different books from different brands and this one is by far my favourite. I would highly recommend to MS3s for their IM rotation.
