6’0. 200 pounds 8” medium girth would xl be right size ?

We are approximately the same size but you are hung better. The pouch was much too big for me, at 5" so I'm better off without it. Hope that helps?

Is there a hole in the rear for entry? I would love to peg my boyfriend while he still has these on!

Yes you will enjoy him eating these! You can peg good. You both will enjoy.
