This strap is adjust from 17 to 21. You can try our ASIN B07ZFCVPX1 which is Adjustable from 22-27 inch, and will be more suitable for tenor sax.

Q: This comes with a plastic hook OR a metal hook and the customer doesn't get to choose?

Mine came with a plastic snap hook. Works well for me.
